As a part of Youth in Action for Sustainable Development Goal Training youth participant Bidhya Basnet under the supervision of Youth Worker Mr. Surendra Bogati implemented a project entitle ‘Reaching Sexual and Reproductive health of Adolescents through Gender lens’ through various milestones at Tarkeswar municipality of Nuwakot District.

Adolescence is the period of physical, psychological and social maturing from childhood to adult hood. Generally, the term “adolescents” refers to individuals between the ages of 10-19 years. Around 1 in 6 persons in the world is an adolescent: that is 1.2 billion people aged 10 to 19 (WHO, 2016) .According to population census done in 2011 adolescents occupy 24% of total population in Nepal. The adolescent’s fertility rate of Nepal is 81 per thousand and median age of first marriage among women is 17.5 years. The is growing recognition that because of a combination of biological, psychological and social factors that adolescents face many different health risks and problems such as sexually transmitted infections including HIV/AIDS, early and frequent pregnancy, substance abuse, accidents and violence. On the other hand adolescents are usually very energetic and receptive to information that pertains to them and are anxious to become more autonomous in their decision-making. There are more than 1000 adolescents friendly center at different public health facilities but utilization of these centers are very low.

Tarkeswar Municipality in Nuwakot district lies 9 km north from Kathmandu, the capital city of Nepal. It is also one among the various districts which was severely battered by the earthquake. As per the 2011 census the population of this municipality was 6000. It has six government school and two private school.

The Gitpurphedi health post was established in the year 2053 B.S as Sub-health post with very minimal health facilities especially focusing on preventive and promotive aspect. Later on with the change in government policy this sub health post was upgraded to health post. It provides maternal, child health and other basic health facilities. This health center do not have adolescents friendly center but its door is always open for the adolescents. It is adhered to the Kalikasthan government school. It has flow of 300 clients per month and nearly 30% of them belong to adolescent’s age group but rarely they come up with the problem related to adolescents.

Volunteer Initiative Nepal has been working in ward no 7 and 8 of this municipality since 2005 AD. So, out of total school we chose one public and one private school for our project. The name of the public school was Kalikasharan secondary school and private school was North Point Secondary School.

 

Overarching Objectives: 

To sensitize the adolescents on sexual and reproductive health through gender lens.

 

Specific Objectives: 

To identify the baseline knowledge of the adolescents.

To deliver the sensitization package.

To evaluate the knowledge after delivering the package

Preliminary phase

During the preliminary phase of the project, coordination and communication was maintained with Volunteer Initiative Nepal, schools and health centers. . Through the help of youth volunteers, site selection was done. Communication was maintained with both the service providers as well as school teachers via telephone, focus groups and one to one meet. Likewise, a field visit was also made in order to understand the real scenario of the adolescents, especially in the context of their gender roles and sexual and reproductive health. The adolescents view on their own body and sexual and reproductive health was taken. Detail study of their curriculum was also made so that our sensitization package do not go beyond their level of understanding. These activities were also an essential prerequisite for the further process of the project. This phase was done during the October month of 2016.

Implementation phase

The preliminary phase set the foundation for the implementation phase of our program. As per the suggestion made by the school teachers and service providers, sensitization package was developed which include the following most known contents:

 

Concept of Gender and Sex

Reproductive health and its nine components

Sexual and reproductive health rights

Relationship between Adolescents and health providers/Adolescents friendly center

Importance of understanding sexual and reproductive health through gender lens

Pre-test questionnaire was set covering all the contents ( annex I).

Nepali language was decided to be used in public school and mixed language ( English and Nepali) in private school. All other trainers were trained on the package.

 

The program was scheduled on 27th November, 2016. As student of class eight, nine and ten were to be included we were given only 2 hr per

class. The session began with general description of program objectives along with brief introduction to Volunteer Initiative Nepal and A.C.T.O.R. Enviroment was made adolescents friendly by using various teaching learning approaches like games, quiz, making them draw on the board and addressing their queries.

The session was led by each of the youth volunteers, including Bidhya Basnet and Surendra Bogati in each of the class. The adolescents also shared about being at ease while asking about their queries to the facilitators. The adolescents shared, “We were taught about the reproductive system before but we were not given its concept through gender perspective. I like the session very much as session is very much effective and we were given information on more practical way. We felt comfortable on putting our queries on anything and clear our confusion”.

Program evaluation: As we lack sufficient time and in order to make adolescents friendly Mood-meter chart was used as a tool for immediate program evaluation. the participants were asked to select the mood drawn on the chart, based on how they found the program to be i.e. whether it was ‘Good’, ‘Satisfactory’ or ‘Poor’. 85% of the participants evaluated that the program was good and 15% evaluated as satisfactory.

Knowledge Evaluation: After completion of the program post test was done in order to compare with their pretest knowledge. During pretest score of public school was 69.3% and private school was 62%. Similarly posttest score of public school was 75.6% and private school was 89.14% Difference between pre and post knowledge of public and private school was 6.3% and 27.1%.respectively.

Long term Evaluation: Long term evaluation could not be carried out due to limited time. So after three month we will be assessing the flow of adolescents utilizing the health services whenever they have need.

Conclusion – Sensitization program for adolescents on Sexual and reproductive health turned out to be a successful initiative with active participation.The session content was easily understandable as it was based on baseline information obtained after series of interaction with the providers, school teachers and adolescents. Different method media used during the session helped them in becoming more receptive towards what was delivered.

 

Major challenge during the project implementation and the way I managed:

 

Time management was a major challenge as we were abide by the rules and regulations of our own job. But however we managed by delegating the responsibilities.

Site election was also great challenge as we had no nay budget and the expectation from training sites were high. So, we selected the site where our organization had a good coordination.

Working without fund was very difficult. So, we manage some amount of fund within our own and also by maintaining good coordination with the school.
